I see patients who have been on chemotherapy for two or three cycles and something does not feel right to them. They are not sure whether the protocol is correct. They have heard that their cancer type might have a targeted therapy option but nobody has tested for it. They wonder whether the treatment is being monitored closely enough. They come to me for a second opinion on treatment. This is not betrayal of their first oncologist. This is intelligent self-advocacy.
A second opinion on a cancer treatment plan is different from a second surgical opinion. In surgery, the question is often: do I need this operation? In medical oncology, the questions are: is this the right protocol for my specific cancer type and subtype? Has the tumour been tested for targetable mutations before chemotherapy was started? Is the treatment sequence evidence-based? Is the monitoring plan adequate? These are the questions I answer when reviewing a treatment plan.

Targeted therapy may be more appropriate than standard chemotherapy for your cancer type.
If cancer is not responding to treatment after 2 to 3 cycles, the protocol should be reassessed. Poor response is information. It means either the protocol needs to be changed or the diagnosis needs to be reconsidered.
Untreatable is sometimes a clinical judgment rather than a biological fact. A second opinion from a different oncologist reviewing all available information may reveal options that were not apparent to the first assessor.
I take this seriously. Patients sometimes sense that something is not right before the test results confirm it. If you have a persistent concern about your treatment, a second opinion is warranted regardless of whether you can articulate why.
Pathology report including receptor status and molecular testing done, current treatment protocol, scan results, treatment response assessments. I review everything independently before making an assessment.
Is the diagnosis complete including all relevant molecular testing? Is the protocol evidence-based for this specific cancer type and subtype? Is the treatment sequence appropriate? Is monitoring adequate? Are there alternatives that should be considered?
A clear, independent assessment. If the current plan is correct, I will confirm it and explain why. If a different approach would serve you better, I will explain that clearly. I do not give ambiguous answers.
